Alcohol & Tobacco License Applications
Businesses that wish to sell alcohol beverages or tobacco / electronic vaping products must obtain the appropriate license. All state application forms are available through the Wisconsin Department of Revenue (DOR).
How to Apply for a Liquor License:
- Complete the Wisconsin Alcohol Retail License application forms. Common forms include:
- AB-100 (Business information)
- AB-101 (Agent appointment, required if corporation/LLC)
- AB-102 (Multiple agents, if applicable)
- Submit completed forms to the Village Clerk located at 125 Depot Street or email to info@vi.johnsoncreek.wi.gov.
- Pay the required license fees due at the time of application.
- Applications are reviewed, and background checks may be conducted.
- Licenses must be approved by the Village Board before issuance.
Operator’s License (Bartender License)
An Operator’s License is required for any individual serving or selling alcohol on behalf of a licensed establishment. Complete the Operator License Application and submit it to Village Hall at 125 Depot Street or info@vi.johnsoncreek.wi.gov with applicable fee.
How to Apply for a Tobacco & Electronic Vaping License:
- Complete the Wisconsin Tobacco Retail License application forms.
- Submit completed forms to the Village Clerk located at 125 Depot Street or email to info@vi.johnsoncreek.wi.gov.
- Pay the required license fees due at the time of application.
- Applications are subject to review and approval prior to issuance.
